eClinicalWorks® is taking advantage of HIMSS12 to announce care coordination for Accountable Care Organizations (ACOs) andPatient-centered Medical Homes (PCMH). Tied directly into the electronic medical records system, this tool will enable care coordination and reporting functionalities essential to these healthcare models.
eClinicalWorks Care Coordination Portal gives organizations the tools necessary to comply with regulations and optimize efficiency. Tying this functionality into the EMR, eClinicalWorks EMR or another, gives users greater ability to effectively provide enhanced care in this environment. Built on eClinicalWorks Electronic Health eXchange (eEHX) private health information exchange, this portal includes:
- Care plan documentation for care management;
- Patient engagement via text, voice and e-mail;
- Home-monitoring device interfaces;
- Transition of care plan management; and
- Integration with eClinicalWorks Community Analytics.
