New Jersey’s Horizon Health Center Selects eClinicalWorks for Care Ooordination
eClinicalWorks has announced that Horizon Health Center, a Federally Qualified Health Center (FQHC) serving Jersey City, is expanding its relationship with the company to include eClinicalWorks Care Coordination Medical Record™ (CCMR) for advancing its Accountable Care Organization (ACO)-related objectives, coordinating care across the continuum as well as evaluating population health and quality improvement activities. An eClinicalWorks customer since 2008, Horizon Health was the first FQHC in New Jersey to implement electronic health records.
“Horizon Health takes a holistic approach with treating patients and looks to continuously improve the quality of care,” said Marilyn Cintron, CEO of Horizon Health Center. “We want to connect the specialists, primary care providers, hospitalists and the family at home to have real, seamless communication. eClinicalWorks has the technology and understands our requirements as we pursue Accountable Care Organization goals, while working with us to achieve them.”
“Horizon Health searched for a community-wide platform to raise the level of care,” said Arlene Simon, COO and CIO of Horizon Health Center. “eClinicalWorks CCMR was the only solution that had all of the pieces we required. The company helps us scale quickly and creates pathways to coordinate care between all the stakeholders. Using this solution will allow us to look at all the data on a community level and make decisions based off of that information.”
Horizon Health uses the eClinicalWorks EHR software, which is enhanced to meet the stringent reporting and billing requirements that apply to FQHCs, ensuring there is no interruption in the funding that supports its providers’ work. Since implementation, Horizon Health has received Primary Care Medical Home Certification by The Joint Commission and nearly 1/3 of its patients are actively communicating with their medical providers using the eClinicalWorks Patient Portal.
“Horizon Health continues to be at the forefront of technology,” said Girish Navani, CEO and co-founder of eClinicalWorks. “Extending their use of EHRs and patient engagement tools by utilizing eClinicalWorks CCMR with closed-loop analytics will enhance decision-making at the point of care and help coordinate care across the continuum.”
eClinicalWorks CCMR is designed to provide visibility into a patient’s care across all settings, facilitate smooth transitions of care, engage patients and providers in preventative care, improve outcomes among populations and reduce costs. A vendor-neutral, open network securely connects ambulatory EHRs with hospital in-patient systems and payor claim feeds, including Centers for Medicare & Medicaid Services (CMS), to provide cost and utilization for services delivered to patients.
Included in CCMR, eClinicalWorks Community Analytics™ uses closed-loop analytics to deliver reporting, alerting and messaging capabilities necessary to manage Population Health. Dashboards can be shown at the community level to determine and monitor adherence of clinical process and outcomes, for including medication recalls and chronic disease management. Care coordination and analytics work together for a collaborative care framework.
